Benefit Summary

Medical Insurance
Group medical insurance is offered through Care First to all eligible employees and dependents with three plans to choose from. BlueChoice HMO requires the selection of a Primary Care Physician (PCP), has no deductible, $20-$30 co-pays, and includes an annual vision exam. BlueChoice Open Access, Opt Out does not require the selection of a PCP, has no deductible, $20-$30 co-pays, and include an annual vision exam. The third option is HMO Open Access with a Health Savings Account (HSA). This plan has a $1,200 individual or $2,400 family deductible, $20-$30 co-pays. For employees enrolled in the HSA plan Walden will contribute $100-$200 monthly into a flexible spending account to cover the deductible cost. All participants will also be enrolled in Care First's prescription drug plan.

Dental Insurance
Group dental insurance is offered through Care First for all eligible employees and dependents. This plan covers 100% preventive, 80% basic care, & 50% major care services. There is an annual $50 deductible ($150 family max.) for basic and major care services.

Employee Medical Reimbursement Fund
Walden offers eligible employees that have completed one full plan year of service (June 1-May 31) an employer-funded benefit plan that reimburses employees for out of pocket medical expenses.

Holiday Leave
Walden observes 9 paid federal holidays annually: New Years Day, Martin Luther King Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Thanksgiving Day, Christmas Day. In addition Walden also observes the following holidays: Presidential Election Day (once every 4 years), Christmas Eve, & the employee’s birthday.

Annual Leave
All full-time employees will accrue paid annual leave based on their years of service starting with 10 days annually. Part-time employees working 20 hours a week or more will accrue prorated leave.

Sick Leave
All full-time employees will accrue paid sick leave based on their years of service starting with 7 days annually. Part-time employees working 20 hours a week or more will accrue prorated leave.

403B Retirement Plan
All eligible employees may participate in the 403B retirement plan immediately. Employees may contribute up to the IRS maximum limit each year. Walden will match a percentage of the employees contributions based on years of service (ranging from 3%-7%). Employees with 3 or more years of service will be 100% vested.

Disability Insurance
Walden offers short-term and long-term disability insurance to eligible employees after one year of service. Short-term disability is available at 60% of your weekly salary to a weekly maximum of $1,000 up to 13 weeks. Long-term disability is available at 66.67% of your weekly salary to a monthly maximum $5,000 beginning after 90 days of continuous disability.

Basic Life Insurance & Accidental Death & Dismemberment
Walden provides eligible employees after one year of service an employer paid term life benefit. Benefit amounts are based on annualized salary. This plan also offers an accidental death and dismemberment feature which will doubles the pay-out amount in the event your death results from an accident.
